Your Appointment

For timely service, please arrive at least 10 minutes prior to appointment time. Please bring your insurance card and drivers license or picture id, a list of current medications if any, and payment for services.  

We accept Cash, Money Order, Visa, MC, Discover and AMEX.  At the present time we only accept Personal Checks for copay & co-insurance amounts not exceeding $50.00.  No split tender is allowed regarding checks. We do reserve the right to refuse any personal check. If you need additional clarification regarding our check policy please contact our office prior to your appointment.

If you have a Flex Spending account we will try to process your Flex Card as payment for Services.  If for some reason our terminal will not accept your Card you will be required to use another method of payment and we will provide you with the necessary documentation for your flex account.
