According to the Case Management Society of America, "Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes."
I used this definition as the foundation for developing my patient advocacy services. My focus is to help patients and their families build the confidence they need to successfully navigate their health care challenges. By providing a caring informative approach to the people I serve I'm able to reduce the anxiety they have about managing their chronic illness, accessing the community services they need, and talking to the providers involved in their care about their concerns.
My Care Management Consulting will include the following services:
I will meet with you and your loved to evaluate your needs. Then we'll design a care plan according to your determined needs. This may include: Providing education and support related to managing a chronic illness, analyzing insurance coverage and benefits, supporting your concerns about a hospital stay, assessing Durable Medical Equipment and Service needs, establishing care with a Provider, and assisting with the transition to different levels of care offered through various housing options.
Conduct ongoing coordination of services until the care plan goal is achieved, accompany clients to provider appointments when necessary, and make onsite facility visits after a placement to ensure the safety and satisfaction of my clients.
